Referral Process
Most of our referrals come from the Letchworth schools under the partnership however there may be instances where professionals from our partner services and organisations can make referrals into our service. The children must be attending a Letchworth school due to how our service funding is provided.
If you identify a family who is in need of support, please ensure you have referred to the tiered approach pdf document outlining what falls within our remit of work and where possible other school based support and interventions have been tried first.
If you decide a family is suitable for our service and you have had a discussion with the family and obtained consent, please complete the LFST referral form./docs/Professional_Referral_Form_October_2024.docx
Once completed, referrals are to be sent to our dedicated referrals email address which is referrals@letchworthfamilysupport.co.uk. Once the referral has been received you will receive confirmation.
The referral form will be triaged by a member of the team and there may be instances where a case discussion is booked in with the referrer for additional information. Contact will then be made with the family.
Should we feel the referral does not meet our criteria we will discuss this with the referrer and offer signposting and/or guidance of where we feel the case may sit best.
Due to our allocated schools system, you will always have a point of contact from the team to discuss any questions you have relating to referrals.
